A patient with type 2 diabetes who was prescribed exenatide in 2005 — Byetta, the drug built from the Gila monster’s venom peptide — learned the rhythm fast. A shot before breakfast. A shot before dinner. Every day, indefinitely. The lizard’s gift, the subject of the previous installment (CS-OZEMPIC-01), had been a genuine breakthrough: it proved that a molecule could mimic the human hormone GLP-1, switch on the same receptor, and survive in the bloodstream long enough to be useful — where the human hormone itself lasts about two minutes before an enzyme called DPP-4 cuts it apart. Exendin-4 lasted hours. That was the difference between a molecule and a medicine.
